Membership & Payment Disclaimer 

This practice operates on a membership-based, self-pay model. Membership fees cover office visits and provider access as outlined in the membership agreement. 
Laboratory testing, imaging studies, medications, and services provided outside this practice are not included and may result in additional charges. 

Insurance is not required to participate in this practice; however, patients are encouraged to maintain health insurance coverage to assist with costs related to hospitalization, emergency care, specialty care, medications, laboratory testing, or imaging.
